How to Graduate

During the quarter you are completing your degree requirements, you should begin to plan for graduation. Start by e-mailing the department academic coordinator to indicate you intend to complete your degree that quarter. You must have all transfer work recorded on your transcript and any approved substitutions to your program noted on your degree checklist in your department student file.

If you have received federal financial aid while at OGI, by law you must have a Financial Aid Exit Interview within 30 days of your last OGI class. Contact OHSU's Financial Aid Office at (503) 494-7800, (800) 775-5460 or finaide@ohsu.edu for more information.

If you are completing a thesis degree, follow the Thesis Publication Flowchart and adhere to the guidelines found in the Thesis Style Guide. Before approval by the EPC, the thesis must be submitted to the OGI Library for review and approval. Consult the library resources well in advance to avoid unnecessary delays in your degree.

These are the three forms that neeed to be completed in order to graduate:

1.Recommendation for Granting of Degree

After you have completed the academic requirements for your degree, a "Recommendation for Granting of Degree" form, signed by your Department, must be submitted to the Department of Graduate Education in order to award your degree. It is your responsibility to ensure this form has been initiated. Since your name on the form will determine how your name is printed on your diploma and the commencement program your involvement in this process is important.

After completing the top portion of this form, submit it to your department�s academic coordinator. After your department has verified your degree requirements and signed the form, they will forward it to the Department of Graduate Education. Once approved by the Department of Graduate Education, it is brought to the next monthly EPC meeting where it is reviewed and voted on. OGI awards degrees monthly and the date of your degree is the date of the EPC meeting.

2.OGI Exit Form
In addition to theDegree Recommendation form, you must fill out and submit the "OGI Exit Form" to the Department of Graduate Education. Obtain the required signatures from your department and central administration to indicate that all of your nonacademic obligations have been met. Because you must return your card key as part of this process, this form should not be submitted until just before you leave OGI.

3.Learning Outcomes Assessment Survey
OGI has an on-line "Learning Outcomes Assessment Survey" that gives you the opportunity to evaluate your OGI experience. Completion of this survey is required prior to receiving your diploma. You are encouraged to provide honest and candid feedback so that OGI can improve the educational experience of present and future students.

The school-wide Commencement and Hooding Ceremoniesare held annually in June. All students who earned a degree during the previous academic year or who expect to complete their degree requirements by the end of summer quarter are encouraged to attend this celebration to honor their accomplishments at the ceremony.
