All full-time, matriculated students are eligible for group student medical insurance through the OGI School of Science & Engineering. As an eligible student, you must elect the OGI School student medical insurance plan or provide proof of other acceptable medical coverage in order to waive the OGI coverage. Insurance waivers must be on file in the Graduate Education office by the 15th calendar day of the term in order to waive the OGI coverage. Coverage is also available for your dependents (if you are enrolled) for an additional cost.
OGI's student medical insurance is in effect during the following times:
- Fall quarter: October 1 through December 31
- Winter quarter: January 1 through March 31
- Spring quarter: April 1 through June 30
- Summer quarter: July 1 through September 30
Insurance needs outside of these dates are your responsibility regardless of your registration status that term. Please plan accordingly.
OGI offers two Kaiser medical plans, the HMO plan and the Deductible plan. The plan design and quarterly cost for these two plans are different. Further details on these two plans and enrollment forms are available from the OGI Graduate Education Department.
Kaiser HMO is the base medical plan, providing the lowest out of pocket cost of care with no deductibles. It offers a $20 office visit charge, 100% coverage for lab and x-ray services, 100% coverage for hospitalization and a $15 co-payment for generic prescriptions or $30 for brand name prescriptions. Under this plan you must receive all care through a Kaiser Permanente doctor. Additional coverage details are available.
Kaiser Deductible is a lower premium, lower benefit option. This plan has a $250 annual deductible to be paid by you before the insurance covers any medical costs. After the deductible is paid, it offers a $15 office visit charge, 80% coverage for hospitalization and a $15 co-payment for generic prescriptions or $30 for brand name prescriptions. Under this plan you must receive all care through a Kaiser Permanente doctor. Additional coverage details are available.
Kaiser Optional Dental provides dental coverage with no deductible. It offers a $15 office visit charge with no additional charge for preventive, diagnostic, basic restorative and oral surgery services. Periodontics, endodontics, major restorative services and removable prosthetics have an additional copay charge depending on the service. Under this plan you must receive all care through a Kaiser Permanente doctor. Additional coverage details are available.
Continuing Coverage:
If you are covered on the OGI student medical insurance plan and loose eligibility (you graduate, go to part-time status or take a leave of absence) you may continue the insurance coverage through COBRA. If your eligibility changes, please contact the Chelea Holdt, holdtc@ohsu.edu, in the Graduate Education Department for further information about COBRA.
Quarterly Premium Costs:
The quarterly premiums for your OGI insurance coverage will be charged to your student account when you register as a full-time, matriculated student. The quarterly premiums for the plan year from July 1, 2007 to June 30, 2008 are:

Another benefit
the OGI School of Science & Engineering provides all full-time, matriculated students and their family members at no cost is a Student Assistance Program (SAP) offered through a company called eni. This program provides confidential, professional support exactly when you need it - day or night, year around. It can help resolve personal concerns and situations that can affect your well-being or academic performance. These concerns may include relationship problems, sexual orientation, alcohol and substance abuse, stress, eating disorders, emotional, financial, and legal problems. The SAP offers an immediate assessment of your needs by a licensed professional, personal support, referral (if needed) to the appropriate level of treatment, interface with college or external resources and appropriate follow up. It's a program just for you and your family; you can trust that it is totally private.
