Application Procedures
For degree programs, the following items must be submitted:
- Completed OHSU Department of Science & Engineering application form. The application is valid for one year from date of submission. A separate application is required for each program.
- $65 nonrefundable application fee per application. The application fee must be paid online at the time the application is submitted using a Mastercard or VISA.
- Official transcripts from each college or university attended. The transcripts must arrive in a sealed envelope and should be mailed directly to Graduate Education. Transcripts must be in English or accompanied by a certified English translation.
- Three letters of recommendation -- each accompanied by the recommendation letter form and signed by the author. Letter writers should mail their letters directly to Graduate Education, or give them to the applicant in sealed envelopes and signed on the back flap.
- Official GRE scores (if applicable, see division-specific application requirements) sent to us directly from the Educational Testing Service (ETS). IMPORTANT NOTE: The institutional code for DSE (formerly OGI) is 4592. DSE does not have minimum GRE score requirements. The GRE is one component of the application. Admissions Committees look at all elements of the application -- statement of purpose, letters of recommendations, transcripts, GPA, research, TOEFL and GRE scores, etc. -- to form an opinion of whether the applicant would be a successful graduate student here, or not.
- Official TOEFL scores (if applicable), sent to us directly from the Educational Testing Service.
For certificate programs, the following items must be submitted:
- Completed the Certificate Program application form.
- $25 nonrefundable application fee, which is valid for one year and cannot be waived or deferred. Once you have received e-mail confirmation that the Graduate Education office has your Certificate Program application form, the application fee can be paid with a credit card by calling 503 494-8243.
Completed applications, transcripts and other application materials should be sent to:
